About Tesamorelin

It is the full 44-amino-acid growth hormone releasing hormone (GHRH) with a trans-3-hexenoyl group added to the front. That cap protects it from the enzyme DPP-4, so it lasts longer than natural GHRH.

Theratechnologies developed it, and the FDA approved it in 2010 as Egrifta for reducing excess abdominal fat in people with HIV-associated lipodystrophy.

Tesamorelin research so far

Falutz and colleagues reported in the New England Journal of Medicine in 2007 that 26 weeks of tesamorelin cut visceral fat by about 15% compared with placebo, while raising IGF-1.

Later studies looked at liver fat in people with HIV, where tesamorelin reduced liver fat and slowed fibrosis progression in a trial published in The Lancet HIV in 2019.

Tesamorelin mechanism of action

The peptide binds the GHRH receptor and raises growth hormone and IGF-1. The hexenoyl group on the front end stops DPP-4 from cutting it, which makes it far more stable than natural GHRH.

Its best-documented effect is on visceral fat, the fat around the organs, which is what the Egrifta approval was based on.

Reconstituting Tesamorelin

For the 5mg size, here’s the concentration you get with common volumes of bacteriostatic water:

• 1mL of water: 5mg per mL
• 2mL of water: 2.5mg per mL
• 3mL of water: 1.67mg per mL

Bring the vial to room temperature, add sterile bacteriostatic water against the glass and swirl gently. It should go clear within a minute or two. Shaking only makes foam.

Store sealed vials in the freezer. Once you’ve added water, keep the vial at 2 to 8°C and plan to finish it within roughly a month.
